At Activa Management Services, LLC and its affiliates ("AMS") and Activa Mutual Fund Trust maintaining your trust and confidence is a high priority. Activa Asset Management, LLC, Activa Benefit Services, LLC and Activa Travel Services, LLC are all related by common control or ownership with Activa Management Services, LLC and all of these entities may be referred to in this Notice as "Affiliates", "we", "our", and "us". Activa Mutual Fund Trust is provided services by Activa Asset Management, LLC. References in this policy to "you" or "your" also includes your spouse, or other family members using AMS service products. Some of our sites contain links to other sites whose information practices may be different than ours. Visitors should consult the other sites' privacy notices as we have no control over information that is submitted to, or collected by, these third parties.

Information We Collect

As part of providing you with our products and services, we may obtain Customer Information about you, including:

Former Customers
